Changes to Medicaid and State 7 Day Follow-up Performance Measures

Effective 10/01/2018 for all Medicaid and State Contracted Providers:

Re: Follow-Up After Discharge From Community Hospitals, State Psychiatric Hospitals, and Facility Based Crisis Services For Mental Health and Substance Use Disorder Treatment Performance Measures

Change: Services provided on the same day of discharge are no longer counted as a follow-up service towards these measures. Individuals must receive a follow-up service that occurs 1 – 7 calendar days following the discharge date. These changes are based on changes to the Medicaid and State performance measurement guide and consistent with HEDIS 2018.
