Rescheduled Appointment Improvement Project

Alliance has been conducting a quality improvement project to increase the number of individuals who show for care after a referral from the Access and Information Center. We have heard, anecdotally, that a large number of the callers reschedule their appointments. We would like to collect data on how frequently this is happening and are asking for your help. If you receive a referral from our Access and Information Center and the caller or your agency reschedules that appointment, please insert the word “rescheduled” in the note section of the referral that you receive.
